Why didn't it give as options:

[ ]US$0 [ ]US$5 [ ]US$10 [ ]US$15
[ ]US$20 [ ]US$25 [ ]US$1000*

The survey question as written forces you to either agree to some fee or give up your membership if you don't believe in a fee. There are many good (and I believe valid) reasons for opposing a fee. A typical ICANN survey leading question routes around them.
